Once you are initially referred to our office by your general dentist, or once we enter you into our computer system as a new patient, you will automatically receive your new patient forms by either text or email (whichever you prefer). This way, you can have this information already filled out before you arrive for your appointment.

If you are a returning patient, we my forward you some briefer patient update forms to get back to us before your appointment. If you prefer though, you can still fill out your initial paperwork when you arrive.
